Wellness Monitoring Visits
Under the supervision of the Nursing Supervisor, an RN or LPN will periodically evaluate the client’s care. They will visit the client to ensure that the daily care provided by the home care worker is being carried out as directed, and that treatment is being administered in accordance with the patients POC (Plan Of Care). The RN or LPN may set up, organize, and take note of the client’s medications. They will also to take the clients temperature, pulse, blood pressure and other vital signs, as well as observe the client, in order to detect deviations from the normal baseline and assess the overall client condition. Following the visit the RN or LPN will notify the physician and case manager of the client’s condition and reaction to drugs, treatments, and significant incidents that occurred during the visit.
