Nursing Evaluations

Under the supervision of the Nursing Supervisor, an RN or LPN will make an initial evaluation visit to each client.  They will explain the procedures and treatment to the client in order to establish cooperation and understanding, while soothing any apprehension the client may have.  The RN or LPN will initiate the clients POC (Plan Of Care) and note any necessary revisions needed in the care to be provided.  The nurse will also record the client’s vital signs to establish a baseline of health and note any deviations in the future visit.  Lastly, the RN or LPN will make an assessment of the overall condition of the client so that the visiting care providers are able to properly monitor and provide care.
