TCS (Transitional Coordination Service) works with a patients MCO (Managed Care Organization) to help transition individuals out of institutional living situations, such as Skilled Nursing Facilities, and back into community based living in their own residence.
This is accomplished by utilizing MFP (Money Follows the Person) which is a state grant given to help residents of qualified institutional settings (Nursing Homes, Skilled Nursing Facilities, and institutions for people with developmental /physical disabilities) move back into the communities to live.
Alliance will work with the patients and the MCO in order to obtain MFP grants, find living quarters and help shop for home furnishings such as a mattress, furniture, towels, etc and assist the patient with all aspects of transitioning into their new home.
This program is specifically dedicated to helping people regain their independence and create a home for themselves.
